Hello,
 
Any ideas on what the current thinking is out there regarding Metformin
in renal impairment? A quick look on Medline pulls up plenty of
references calling into question the validity of the traditional 30 -
50ml/min cut off. A Cochrane review lately found no increased risk with
patients that had no contraindications to metformin and one trial in the
Eur J of Int Med included patients with some contraindications and found
no increased risk.
 
I have got the impression that there is more of a move to pushing the
boundaries a bit more with the creatinine clearance cut off even to the
extent of telling patients to stop taking their metformin if they start
developing signs of lactic acidosis!! 

Any enlightenment to the current prescribing patterns/thoughts
gratefully received.
