I have had a couple of enquiries about the page not allowing you to complete
the survey.

 

The survey "remembers" the IP address of the computer. So if someone else
has used the computer to access the webpage you will not be allowed to
complete the survey. Please find another computer if possible. If not,
contact me and we should be able to arrange another way to complete it.

 

No passwords or pass-names are required. While this is listed on the survey
form it is for the organiser of the survey to gain access to the data. Just
fill the survey in and hit "click answer" button at the bottom of the form.
