This email is inform you and invite your input into a project which has been established to address the information needs of Aboriginal and Torres Strait Islander peoples with heart failure.

 

This project is part of the Joint Heart Failure Program, a collaboration of the Heart Foundation, National Prescribing Service and National Institute of Clinical Studies/National Health and Medical Research Council, and is one component of a broader implementation plan to improve the adoption of evidence-based management of chronic heart failure.
